Pacing in heart failure: the benefit of resynchronization.

چکیده
Cardiac resynchronization therapy involves pacing of the left ventricle alone or in concert with the right ventricle within a certain range of atrioventricular delay. It may help patients with systolic heart failure and conduction disturbances by optimizing myocardial performance.

Chronic Right Ventricular Pacing in the Heart Failure Population
PURPOSE OF REVIEW We review the trials that have demonstrated potentially harmful effects from right ventricular (RV) apical pacing as well as reviewing the evidence of alternative RV pacing sites and cardiac resynchronization therapy (CRT) for patients who have heart failure and atrioventricular (AV) block. RECENT FINDINGS The role of CRT in patients with AV block and impaired left ventricul...
متن کاملLeft ventricular endocardial or triventricular pacing to optimize cardiac resynchronization therapy in a chronic canine model of ischemic heart failure.
Cardiac resynchronization therapy (CRT) is a proven treatment for heart failure but ~30% of patients appear to not benefit from the therapy. Left ventricular (LV) endocardial and multisite epicardial [triventricular (TriV)] pacing have been proposed as alternatives to traditional LV transvenous epicardial pacing, but no study has directly compared the hemodynamic effects of these approaches. Le...
متن کاملEnhancing Cardiac Resynchronization Therapy for Patients with Atrial Fibrillation: The Role of AV Node Ablation.
Cardiac resynchronization therapy (CRT) has evolved as an effective therapy for patients with congestive heart failure (CHF) and ventricular dyssynchrony, currently defined as a wide QRS on the electrocardiogram. While multiple randomized controlled trials have confirmed the favorable effects of CRT on mortality and heart failure symptoms for patients in sinus rhythm, only recently observationa...
متن کاملlectrocardiographic imaging of cardiac resynchronization herapy in heart failure: Observation of variable lectrophysiologic responses
BACKGROUND Cardiac resynchronization therapy (CRT) for congestive heart failure patients with delayed left ventricular (LV) conduction is clinically beneficial in approximately 70% of patients. Unresolved issues include patient selection, lead placement, and efficacy of LV pacing alone. Being an electrical approach, detailed electrical information during CRT is critical to resolving these issue...
متن کاملQuadripolar Leads in Cardiac€Resynchronization Therapy
Ma Despite the benefit of cardiac resynchronization therapy (CRT) in patients with heart failure and conduction delay, a considerable number of patients do not respond substantially. Left ventricular lead position is an important factor in response, restricted by the patient’s specific anatomy and local pathophysiology. Quadripolar leads could enhance response to CRT, offering 4 pacing location...
